References:
- Required minimum of 1 year experience in your field
- Must provide names, addresses & phone numbers of your previous clients, registries, agencies, or employers for at least the last five years.
- Your references will be verified.

- Driver's License or a picture ID if you do not have a driver’s license
- Social Security Card
- Work Authorization (if appropriate)
- CPR Card
- License / Certificate:
- RN's & LPN's - Florida State License
- CNA's - Florida State Certificate
- HHA's - minimum 75 hour HHA Certificate which shows the curriculum stipulated by State of Florida Administrative Code, 59A-8.014
- Physical within 6 months signed by medical doctor stating "Free of communicable disease and of good health sufficient to provide services to persons with compromised health."
- Chest X-Ray within last 6 months or Mantoux Method Tuberculin skin-test within 6 months with negative results. This must be signed by the doctor or person administering test.
- HIV Original Certificate:
- 3 hours for RN's & LPN's
- 2 hours for CNA's & HHA's
- HIV Update:
- 1 hour for RN's & LPN's
- 1 hour for CNA's & HHA's
- Alzheimers Update:
- 1 hour for RN's & LPN's
- 1 hour for CNA's & HHA's
- Assisting the client w/self medication:
- CNA's & HHA's only
- Prevention of Medical Errors:
- RN's, LPN's & CNA's
- Domestic Violence Certification:
- RN's, LPN's, CNA's & HHA's
- $30.00 Processing Fee for FDLE Background Screening
